We wrapped up our 2019 Educational Outreach events with a December Informational Tour of two Clearbrook Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF-IIDs): a 16-person home and an 85-person home.
ICF/IIDs are a residential option that includes on-site nursing support. Residents either have their own bedroom or share a bedroom with one other resident. Goals are developed for each resident and programming is received in the areas of self-care, academics, community living, self-medication and workshop skills. Admission to an ICF/IID is a federal entitlement, meaning that if an individual is eligible and the facility agrees to serve the individual, he/she is automatically entitled to funding without going through the PUNS selection process. ICF/IIDs are licensed and regulated by the Department of Public Health.
Many thanks to Clearbrook Director of Admissions, Heidi Raymond, for scheduling our visit, and to Tracy Martin for conducting the tour.
Connect to Community Educational Outreach events are offered in partnership with the Northwest Suburban Special Education Organization (NSSEO) and are paid for in part by community funding from Palatine Township. We thank them both for their support!
