Connect to Community, in partnership with the Village of Hoffman Estates Commission for People with Disabilities and the Northwest Suburban Special Education Organization (NSSEO), presents its annual Transition Summit, back in person in 2023!

On the agenda …

Housing Options in Illinois: CILAs, Supportive Housing, ICFs, and more ~ From all the different CILA models (24/7, Intermittent, Host Family, etc.) to Supportive Living, CLFs, ICFs, and SODCs, there are many different housing options for families to consider. IDHS Statewide Housing Coordinator Todd Fuller will explain the differences between these various housing models and will share how they are accessed and funded.

An Introduction to Host Family CILAs ~ Based in Peoria since 1950 and now serving 15 counties, EP!C is a community of families, staff, volunteers, and community members working together to support individuals with intellectual and developmental disabilities and their families. Their Host Homes Program, one of the largest in Illinois, matches adults with I/DD with host families in their community of choice. Host families provide more than just a place to live by offering individualized support, encouraging independence, and providing immersion in the community.

One Family’s Story: Making Independent Living a Reality ~ As families, our greatest hope is to find a living arrangement that maximizes our loved one’s independence and happiness. Sometimes that may mean taking charge and creating one ourselves. Parent Vicki Niswander will share her family’s experience in creating a home for their daughter Annie with support and assistance from Annie’s Microboard and their Independent Service Coordination agency. With her own house and a job in the community, Annie is living the life she always wanted.

Creating Housing Options: The New Futures™ Initiative ~ The New Futures™ Initiative is a step-by-step process that gives families the ability to open doors to supported living options in their own neighborhood. At a time when government housing is at capacity and funding is not increasing, the New Futures™ Initiative serves as a road map to help guide families on their paths toward the housing solutions their loved ones need and deserve. Center for Independent Futures will explain their process, including an individual and family member perspective.

ISC Statewide Housing Navigator Pilot ~ The ISC Housing Navigator Pilot is designed to cultivate and pursue community-based, independent housing options for people with I/DD who are already in the waiver or newly selected from the PUNS list. The ISC Housing Navigator will foster landlord and affordable housing relationships in order to facilitate a match with individuals with I/DD who are interested in independent supportive housing. Miranda Martinicky, the Housing Navigator for Community Alternatives Unlimited (CAU), wll share the details of this interesting new concept.
